• Specialty self-referrals
  Your Access+ HMO plan gives you plenty of choice. You can see a specialist recommended by your Personal Physician or you can choose to self-refer to a specialist within your medical group or IPA (independent practice association). While this self referral feature has a slightly higher copayment, the choice is yours.3
  
  
• Log in for $175 with Healthy Lifestyle Rewards
  Healthy Lifestyle Rewards is an interactive online program that gives you the resources, motivation, and support you need to eat healthy, get fit, manage stress, and more. As a Health Service System Access+ HMO plan member, you can earn cash rewards when you participate in the program. Complete the Wellness Assessment and earn $25 right away. Earn an additional $50 for every 12-weeks of participation (up to 36 weeks) for a total of $175.4 A change in your health can also mean change in your pocket. • Dedicated Member Services
  Your dedicated Member Service Representatives are based here in Northern California and are trained on all the features and benefits of the San Francisco Health Service System Blue Shield plans. • Urgent care away from home
  Through the BlueCard® Program, you and your eligible family members will have access to urgent care across the country and around the world. You can locate a BlueCard provider any time by calling (800) 810-BLUE (2583) or by going to the Find a Provider section of blueshieldca.com.
