    • Performance Profile: By going to the Find a Provider section of blueshieldca.com, you can access quality and efficiency indicators, and patient satisfaction scores for HMO medical groups and hospitals with the Performance Profile.

    Breathe easier with this program based on National Asthma Education and Prevention Program guidelines. It helps you create a set of written instructions to successfully manage asthma symptoms.   • Congestive Heart Failure Program
    This program offers qualified members an at-home monitoring program that tracks patient weight, heart failure symptoms, and medication compliance for members diagnosed with severe heart failure.

  • Coronary Artery Disease (CAD) Management Program
    With a goal of lowering risk and preventing further problems, this program empowers members with heart disease to make the necessary lifestyle changes to help improve their quality of life.

  • Chronic Obstructive Pulmonary (COPD) Disease Management Program
    Members with COPD learn to promote lung health and receive information on how to use medication to help keep symptoms under control and help reduce shortness of breath.

  • Depression Management Program
    This program promotes early identification and treatment of this silent but serious disease. Resources include a Depression & Mental Health Resource Packet that has fact sheets on depressive disorders, frequently asked questions about antidepressant medication, psychotherapy treatment, depression in children, coping with grief, and caregiver stress.

    As a Blue Shield member, you’ll have access to discounts on chiropractic care, acupuncture, massage therapy, and vision services. Some services offered through the discount program may already be included as part of the Blue Shield health plan covered benefits.  Members should access those covered services prior to using the discount program.
Members who are not satisfied with products or services received from the discount program may use the Blue Shield’s grievance process described in the Grievance Process section of the Evidence of Coverage or Certificate of Insurance. Blue Shield reserves the right to terminate this program at any time without notice.
